I hereby request a Free Independent Report from The Card Processing Advisory Service. I understand that the report may identify ways to secure improved card processing rates and I agree that should we choose to avail ourselves of savings identified in the report, then we will pay The Card Processing Advisory Service 25% of the savings.

On receipt of this form, The Card Processing Advisory Service will analyse the card transactions that we have made in the last 12 months and will consult with the Card Processing banks to identify possible savings. The results of this analysis and consultation process will be provided in the Free Independent Report. I understand that I am under no obligation to follow the recommendations contained in this report and that only if I choose to do so will I be liable to pay any fees to The Card Processing Advisory Service.
